Moving to In Vitro Fertilization (IVF) is a major decision. It is completely normal to feel overwhelmed by the idea of daily injections, frequent monitoring, and the emotional weight of the process.
However, IVF is not just a treatment—it is the most powerful diagnostic and therapeutic tool we have. When natural conception, medications, and IUI have failed, you are left wondering why. IVF takes the invisible process of conception out of the dark and brings it into a highly controlled laboratory setting. We do not just hope the sperm and egg meet; we physically put them together, watch the embryo grow, and place it exactly where it needs to be.
Who Actually Needs IVF?
While IVF is highly successful, it is not always the first step. We typically recommend moving to IVF when there is a clear mechanical or biological barrier that simpler treatments cannot overcome.
IVF is the most effective path if:
- Your fallopian tubes are blocked or damaged: If the tubes are scarred from endometriosis, previous surgeries, or infection, the egg and sperm cannot physically meet. IVF bypasses the tubes entirely.
- Severe male factor infertility: If the sperm count is extremely low, or if the sperm have very poor motility or abnormal shapes, IUI will not work. In the IVF lab, we can inject a single healthy sperm directly into the egg (ICSI).
- Diminished Ovarian Reserve or Advanced Age: If your egg count is low, we cannot afford to waste time on low-success-rate treatments. IVF allows us to retrieve and fertilize the eggs you have left before the window closes.
- Unexplained Infertility & Repeated IVF Failures: If everything looks normal but you cannot stay pregnant, IVF allows us to physically see if the issue is with fertilization or embryo development. We can also genetically test the embryos before transfer to prevent miscarriages.
The IVF Process: Step-by-Step
An IVF cycle takes about four to six weeks. It requires dedication, but it is highly structured. You will know exactly what is happening at every phase.
Phase 1: Ovarian Stimulation (10-12 Days)
In a natural menstrual cycle, your body recruits a group of eggs, but only allows one to mature. The rest die off. During IVF, you will take daily hormone injections at home to “rescue” that entire group of eggs, prompting your ovaries to mature multiple eggs at once.
- What it feels like: The needles are tiny (similar to an insulin pen). As your ovaries grow, you will likely feel bloated and heavy, similar to severe PMS. Dr. Lakshmi Priya will monitor you closely with ultrasounds every few days to measure the growing follicles and adjust your medication doses to prevent hyperstimulation (OHSS).
Phase 2: The Egg Retrieval
Once your ultrasounds show the eggs are mature, we administer a “trigger shot” to finalize their development. Exactly 36 hours later, you will come in for the retrieval.
- What it feels like: This is a minor, 15-minute surgical procedure performed under light anesthesia. You will be completely asleep and feel no pain. Dr. Priya uses a thin needle guided by ultrasound to gently draw the eggs out of your ovaries. You will wake up, rest for an hour, and go home the same day with mild cramping.
Phase 3: The Laboratory (Fertilization & Growth)
While you rest at home, the real work happens in our embryology lab. We combine your retrieved eggs with your partner’s washed sperm. If male factor infertility is an issue, we use ICSI to force fertilization.
- Over the next 5 to 6 days, our embryologists watch the fertilized eggs divide and grow into blastocysts (advanced embryos). We will call you with daily updates on exactly how many embryos are surviving and thriving.
Phase 4: The Embryo Transfer
Depending on your specific hormone levels and protocol, we will either transfer a fresh embryo a few days after retrieval or freeze your embryos for a Frozen Embryo Transfer (FET) in your next cycle.
- What it feels like: The transfer is incredibly simple and completely painless. It feels exactly like a routine Pap smear. Using ultrasound guidance, Dr. Priya passes a soft, flexible catheter through your cervix and places the best embryo directly into the optimal spot in your uterine lining.
